We initiated this investigation on January 16, 2018. During our investigation, we also identified an emergent allegation that Ms. Loftus failed to treat her staff with respect. If substantiated, these actions would violate standards summarized throughout this report.
During our investigation, we interviewed 18 witnesses. Witnesses interviewed included the complainant; sixteen personnel with direct knowledge of the allegations; and one subject matter expert. We reviewed Ms. Loftus’ official calendar, approximately 10,000 official e-mails, travel records, leave records, internal DON SAPRO documents pertaining to Ms. Loftus’ travel, and the applicable standards.
We substantiated the allegation that Ms. Loftus wasted Government resources by conducting official travel for primarily personal reasons and by conducting minimal or no official work during other trips as described in this report. We also substantiated the allegation that Ms. Loftus failed to treat her staff with respect. We summarize our conclusions in this Introduction and Summary, and provide the facts and analysis underlying these conclusions.
