Report Of Investigation: U.S. Air Force Reserve 434th Aircraft Maintenance Squadron Grissom Air Reserve Base, Peru, Indiana (Report No. DODIG-2024-132)

We conducted this investigation in response to a complaint alleging that (the Subject), U.S. Air Force Reserve, Flight Chief, Grissom Air Reserve Base, Indiana, restricted (the Complainant) from lawfully communicating with an Inspector General (IG). A preponderance of the evidence indicates that the Subject restricted the Complainant from communicating with an IG. Therefore, we substantiated the allegation that the Subject restricted the Complainant from lawfully communicating with an IG.

Whistleblower Reprisal Investigation: L.C. Industries, Inc. Vicksburg, Mississippi (DODIG-2024-129)

We conducted this investigation in response to a reprisal complaint alleging that L.C. Industries (LCI) took a series of qualifying actions against (the Complainant) in reprisal for making protected disclosures concerning the improper use of Federal funds and that the Complainant’s LCI supervisor had retaliated against her. The Complainant was employed as an assistant store manager at LCI base supply centers in Vicksburg, Mississippi.

Whistleblower Reprisal Investigation: U.S. Army Reserve 11th Expeditionary Combat Aviation Brigade Fort Carson, Colorado (DODIG-2024-128)

We conducted this investigation in response to a reprisal complaint alleging that (Subject), U.S. Army Reserve, 11th Expeditionary Combat Aviation Brigade (ECAB), removed (Complainant), USAR, 11 ECAB, from a deployment to the Middle East that subsequently made them ineligible for a temporary promotion, in reprisal for making protected communications concerning a sexual assault. The Complainant also alleged that the Subject recommended for release from active duty, also in reprisal.

Quality Assessment Review of the Administrative Investigative Operations of the Defense Threat Reduction Agency Office of the Inspector General (DODIG-2024-032)

On November 21, 2023, the Department of Defense Office of Inspector General (DoD OIG) completed a quality assessment review of the Defense Threat Reduction Agency Office of the Inspector General (DTRA OIG). The DoD OIG determined that the system of internal safeguards and management procedures for the investigative function of the DTRA OIG in effect for calendar years 2018 through 2022 complied with the quality standards established by the DoD or with other applicable guidelines.
