Department of Defense Suicide Event Report (DoDSER) Data Quality Assessment

The Department of Defense Suicide Event Report (DoDSER) is the system of record for health surveillance related to suicide ideations, attempts, and deaths. This assessment focused on decreasing the number of “don’t know” responses on suicide death submissions by identifying changes to policy, training, or oversight. We also examined the sharing of DoD medical information with the Department of Veterans Affairs (VA).

Army Personnel Complied With the Berry Amendment But Can Improve Compliance With the Buy American Act

Our audit objective was to determine whether Army personnel complied with the Berry Amendment and the Buy American Act when they purchased covered items such as food, clothing, tents, textiles, and hand or measuring tools. We performed this audit in response to Section 1601 of the National Defense Authorization Act for FY 2014. The act requires the DoD Inspector General to conduct periodic audits of contracting practices and policies related to procurement under section 2533a, title 10, United States Code, the Berry Amendment.

The Services Need To Improve Accuracy When Initially Assigning Demilitarization Codes

This report is one in a series of reports concerning the DoD Demilitarization (DEMIL) program. The audit objective was to determine whether the Services were properly evaluating DoD property and assigning accurate DEML codes when property enters the DoD supply system. We also determined whether the Defense Logistics Agency was validating the accuracy of DEMIL codes when property was sent to the Defense Logistics Agency after being identified as excess.

Task Force to Improve Timeliness of Senior Official Administrative Investigations (Redacted)

At the request of the Deputy Secretary of Defense, the DoD IG led a task force to review and improve the timeliness of senior official administrative investigations. The task force reviewed data, processes, policies, and resources, and examined proposed changes to conduct more efficient and timely investigations. The task force prepared a plan of action consisting of short-, medium-, and long-range goals to implement the task force recommendations.

Evaluation of Matters Related to the Death of Navy Seaman Kyle Antonacci

This evaluation was initiated in response to a complaint to the Defense Hotline. The complaint questioned the integrity of the investigation into the death of Navy Seaman Kyle Antonacci and specifically alleged the Naval Criminal Investigative Service (NCIS) and Department of Navy officials conspired to rule the death a suicide. The objective of our evaluation was to determine whether the NCIS conducted three related criminal investigations (rape, perjury, and death) in accordance with NCIS standards and whether any evidence exists to suggest a conspiracy.

Our objective was to inspect DoD military housing in the Republic of Korea for compliance with DoD and Federal environmental health and safety policies and standards. Those policies and standards include the Unified Facilities Criteria (UFC), National Fire Protection Association (NFPA) codes and standards, National Electrical Code (NEC), and U.S. Environmental Protection Agency (EPA) standards.

Policy Changes Needed at Defense Contract Management Agency to Ensure Forward Pricing RatesResult in Fair and Reasonable Contract Pricing

We reviewed Defense Contract Management Agency (DCMA) forward pricing rate policy and practice for indirect rates for compliance with the Federal Acquisition Regulation (FAR) and DoD policy. DCMA forward pricing rate policy covers the 353 contractor locations where DoD contracting officers use DCMA forward pricing rates to negotiate at least $70 billion in Government sales.
