Report of Investigation: MG Antonio A. Aguto Jr., U.S. Army (Retired) Former Commander, Security Assistance Group – Ukraine (Report No. DODIG-2026-067)

The DoD Office of Inspector General initiated an investigation into U.S. Army Major General Antonio A. Aguto Jr., former Commander of Security Assistance Group-Ukraine, Operation ATLANTIC RESOLVE, on June 6, 2024. This action followed the receipt of three anonymous complaints between May 20 and May 24, 2024.

Fiscal Year 2026 Joint Strategic Oversight Plan For Operation Atlantic Resolve

We are pleased to submit our annual joint strategic oversight plan describing oversight activities for Operation Atlantic Resolve (OAR) and the U.S. Government’s response to Russia’s full-scale invasion of Ukraine. This plan fulfills the individual and collective oversight responsibilities of the Department of Defense (DoD), Department of State (State), and U.S. Agency for International Development (USAID) Inspectors General (IG) under the IG Act of 1978, as amended, and the National Defense Authorization Act (NDAA) of 2024.

The DoD and Department of State OIG Joint Audit of U.S. Assistance Provided in Support of Ukraine Through the Foreign Military Financing Program (Report No. DODIG-2025-168)

The FMF program is part of the security assistance authorized by the Arms Export Control Act that enables eligible partner nations to purchase U.S. defense articles (such as weapons and other equipment), services, and military training. The FMF program follows the Foreign Military Sales (FMS) process. FMF is a source of financing provided to a partner nation through the issuance of a grant (non repayable) or direct loan. FMF funds are transferred to a U.S. controlled account maintained on behalf of the foreign partner and executed by the U.S. Government.

Evaluation of DoD Processes to Provide Repair Parts to Support the Ukrainian Armed Forces (Report No. DODIG-2025-152)

The objective of this evaluation was to assess the efficiency and effectiveness of DoD processes for providing repair parts for U.S. equipment to Ukraine. Based on information we learned during the evaluation, we narrowed the scope to focus on repair parts shipments delivered to the Remote Distribution Center–Ukraine (RDC U) without a requisition. Specifically, we reviewed those shipments to determine whether DoD officials properly accounted for the parts for U.S. provided equipment delivered to the Ukrainian Armed Forces (UAF), in accordance with Defense Security Cooperation Agency (DSCA) Manual 5105.38 M, also referred to as the SAMM.
